Jul 10, 2015 4:26 AM in response to lllaassby alfa78,I didn't speak about "Apple Hardware Test", but "Apple Service Diagnostics v.3S151", which is a test lasting for several hours (2 hours and half in my case)!
This is a grab of "Apple Hardware Test":
And this is a grab of "Apple Service Diagnostics v.3S151":
As you can see, they're very very different!!!
Anyway, some days ago I went to an Apple Store, and at Genius Bar they tested my Mac Mini: after one day I came back and they said me that my Mac hadn't any issues, it was perfect!
Furthermore, if it could be an hardware issue, it means that ALL Mac machines having Intel HD Graphics 4000 as GPU (here the list: About integrated video on Intel-based Macs - Apple Support ) have to be replaced by Apple!!!

Jul 12, 2015 2:42 AM in response to aaronrryanby Bertie64,Same issue here - 2012 i5 2.5...HD 4000 Graphics...
Been going on for a few months now.
TBH It's not a huge issue for me, as others have said just moving an open window around the area seems to clear the issue each time it happens.
It is annoying though and I really hope apple do something about it.
The majority of the other, more important things, on my own list of annoying Yosemite glitches seem to have been addressed, which I guess is good... It's been quite a flakey release though. I hope the next OS X is more reliable from the get-go.

Jul 14, 2015 10:29 AM in response to aaronrryanby StinkyOldC,i have found turning transparency off in accesibilty in system preferences definetly helps a lot. I've had transparency switched on again the past week since upgrading to 10.10.4 hoping the problem would of been resolved but i must say i had forgot how frequently i was getting these graphical artefacts as they have been happening frequently every day since i have switched this setting back on. I have now switched transparency off again the past day and the graphical artefacts are practically non existent. Suggests to me a software issue but I'm no expert. Anyways give it a try

Jul 17, 2015 8:58 AM in response to aaronrryanby [GeorgeT],Been getting them too
I've tried 10.10.4, and even El Capitan public beta - no change. The firmware upgrade released yesterday also didn't help.

Jul 24, 2015 8:32 AM in response to [GeorgeT]by StinkyOldC,well after a week I'm getting a few slight graphical artefacts not as bad as before but no, this firmware update didn't fix it....oh well. will be off to applestore at the end of the year before my applecare runs out

Oct 20, 2015 4:55 AM in response to StinkyOldCby StinkyOldC,well i spoke too soon, saw them again today. although not as regular as before its still an issue clearly. My applecare runs out mid january so will be going to the apple store in the next few weeks to get it sorted as I'm now leaning more towards a hardware fault probably the gpu

Nov 8, 2015 12:49 PM in response to aaronrryanby agin,I upgraded from Mavericks to El Capitan (10.11.1) and have been experiencing sporadic display glitches while watching videos. While I'm watching the video, bands of flashing blocks of color will appear outside of the active window. These artifacts remain on the desktop even after the video is done. Once while watching the video full screen, the title bar kept flickering. This happens in various apps: Firefox and Reeder.
I will turn off transparency as suggested and see if this happens again.

Nov 19, 2015 5:37 AM in response to aaronrryanby jimmymacca,If you press cmd alt and eject all at once, it will switch the display off, then wait a few seconds and then press any key when the screen wakes put in your login password. Hopefully the glitches will be gone. Repeat as necessary sometimes it doesn't do it first time.
This method seems to reset the graphics card, Its not a full fix as it will happen again, but it saves you restarting.

Dec 1, 2015 10:37 PM in response to aginby agin,Update to my video glitching problem:
I installed Macs Fan Control since I thought the problem might be caused by overheating. This actually helped somewhat. I didn't get residual artifacts on the desktop after watching on-line videos, but sometimes I would get some flashing artifacts outside of the active window. I had been experiencing the problem while using Firefox and Reeder, so I checked to see if it would happen with Safari. Surprised to see that the problem did not occur with Safari or Quicktime Player! Unfortunately, I prefer Firefox to Safari. Will have to test with other apps.

Dec 2, 2015 11:04 AM in response to aginby K Shaffer,The fact that at least in your experience, something in the code (mozilla firefox?) appears to
help trigger the graphics issue, may lead one to attempting to suggest this matter to Mozilla
so they can look into the build codes in their Firefox browser. Maybe it has some parts that
do not fair well when in use among other system or application codes; or with the firmware.
There is a feature in Firefox browser to report an issue; however the real proof would be in
the lines of system information relating to whatever is happening in your system. And that is
where this problem may find some resolution, if the right people actually had access to the
unit and the in-situ problem generated by the configuration of software & hardware inside.
{I've not upgraded my Late 2012 Mini i7 quad server beyond as-shipped OS X 10.9(.5) and
when I bought this unit from Apple Store online (clearance/new) hoped that it would be an
older stock model with Mountain Lion, since it shipped with only 4GB RAM. However I have
not done much with the computer, & could have saved $1K by exercising pre-buyer remorse.
Have considered partitioning one of the two factory installed 1-TB HDDs so as to create a
space for later OS X installations, while leaving the original Mavericks installation intact; but
did not hear back on inquiry, if I could do that and get the application upgrades, (as offered
to new computer purchases) installed in the second HDD as well, and so I haven't. A year
ago on 23 December; so I'm considering Applecare plan. Unanswered questions there, and
lack of on-ground support in AK have left me wondering why I bought such a new item; as
I've much previous experience using & repairing older model Macs...}
Anyway, if Mozilla were able to make head or feet out of your situation, given the error message
or line of code from terminal or other internal complication due to the combination of their Firefox
and the later version of OS X in the hardware of your Mac Mini... that would be a rare incidence
of someone noticing a problem exists and seeking a cure for only one model series involved.
As I've said before...
Good luck & happy computing!

Dec 15, 2015 11:18 PM in response to K Shafferby xchomik,First of all - hi .
You actually pointed me to check the FF, thanks. The same problems occure in my Mini 2012 i7. I didn't pay as close atention, but I think it happens only while I'm watching YT in FF, but I will have to check Safari to confirm if this is FF problem or more global thing.
