Q: OSX Yosemite 2nd monitor issue after upgrade

MacBook Pro (Retina, Mid 2012)

NVIDIA GeForce GT 650M 1024 MB

OS X 10.10.3

This worked before the upgrade.

I've seen a few others that seem similar to mine, but this could be unique.   I have a HDMI-->DVI-->Acer AL2216W adapter plugged into my MBP.

When it works:
1.  After reboot

When it doesn't work:

1.  After plugging into any other monitor and no reboot.

All other monitors using a straight HDMI or Thunderbolt adapter work fine.  They work fine without reboots as well.   Oddly the DVI monitor does appear to be "active" as I can drag windows into it, it just doesn't actually light up the screen.   After a reboot, the monitor is active again.   Once I plug into the TV, or an Apple Display, it stops working until another reboot. 

Also, during reboots, I get a message that my computer shut down unexpectedly, which isn't true as I initiated the reboot.
