Q: 1 phone, 4 IP Addresses 3 out of state.  How is this possible?

I Have an iPhone 4s. Google notified me that someone logged into my accounts from unknown devices. I checked the activity logs & noticed unfamiliar IP Addresses & locations, 1 in Nevada, 2 in Arizona & 1 in Calif. (me).  I looked up my cell phone's IP & it matched the one assigned to my device.  I changed my passwords but this is still occurring with my accounts.  I've never been to Arizona or Nevada. How many IP Addresses does this device have, how would my locations & IP addresses do this... Even after I changed my PW's.  Could someone be using a cloned device. I know this is extremely rare but it is still possible.  Any sound explanations would be appreciated.  (I was hacked once before).
