How do I know if I was "hacked"?

I fell asleep with my personal hotspot and wifi on. I woke up to find:

- a web page opened to yahoo finance

- a web page opened to some gspa21.ls.apple acknowledgement page

- My browser was set to private

- my bluetooth was on

- two blank pictures were posted on instagram

- a random app "RF" was the logo was on my screen. I immediately deleted it.

are these signs of being hacked? If so, what do I do?
