Power adapter melted

The 12W power adapter that came with my iPad 3 got so hot that it caught fire briefly. Is this a known issue? Whether it is or not, why should I pay Apple $30 to let them know about it?

Make an appointment at your local apple store, take it in and talk to them about it.

A suggestion....these folks are just doing their jobs, so it's good for you to make it easy for them to help you.

My "local" Apple Store is more than 4 hours away. That's not really a viable option for me, but there's not even a way to report an issue without having to pay them for letting them know about a potentially deadly defect. This adapter started to burn 2 feet away from my head while I was sleeping. If I hadn't woken when I did, I might never have done so at all.
