Is there a question here somewhere?
If your question is how to insert a SIM into the CDMA iPhone 4S, check this link:

EDIT: If it's a Verizon iPhone, it will already be unlocked for use on GSM networks. If it's not, you'll need to contact the carrier to get it unlocked.
Also note that if it is an iPhone 4, it can not be used on any GSM networks at all. The CDMA iPhone 4 does not have a SIM slot or a GSM radio.
my phone is iphone 4s and I'm using this phone from 2years. Suddenly i got this problem
WHAT problem? It is not possible to use it on a GSM network unless you have a SIM in it. It never has been.
I Used this phone for 2years with GSM network and now it is not working with GSM and getting a notification that device is activated on CDMA US network.
What is the EXACT message you're getting and under what circumstances?
I Am getting this "Device is activated on CDMA US network. International roaming requires a sim"
Sounds to me like it's carrier locked and was hacked or jailbroken to unlock it. Something happened that caused you to lose the hacked unlock. If that's the case, you'll have to take it back to the US. Sign up for service with the carrier it's locked to. Maintain service for their minimum required period of time, then request an unlock.
