iPhone 5 camera focus blurred

How to fix the camera focus when blurred? Trying to take pictures of items, such as signs with writing or barcodes, and focus is blurred. Followed the steps in the help section, but will not focus. Tried resetting, tried restarting, tried different app, such as barcode scanner - all still blurred. Help!!
