Woke up to yet another service mysteriously turned off. My Mac couldn't send SMS anymore. Went into iOS Settings and iMessage was turned right off. Same on Mac, even though I made an app-specific password for it a day ago.
As for the iPhone, the settings for Text Message Forwarding are gone completely from the Messages settings.

Even though I signed in to iMessage, apparently you have to go to "Send & Receive", and sign in to iMessage again there. This time it told me to use an app-specific password, which I had to generate and copy through the website it opened in Safari. After that, the forwarding setting is visible.
