deleted app keeps reinstalling when i sync iphone. why ?

An App MC5 was installed onto my ipad and when i was syncing ipad & iphone to laptop it installed it onto phone. i'm not a big game fan so immediately deleted it from both devices. Now every time i sync my phone it reinstalls itself or tries but there's never enough free space. The problem is i cannot find any connection to the app at all on either device or the cloud or laptop. Could someone please help me to delete this app for good.
