I use my old iPhone 4s for music in my car but today when I added some new music it loaded all of the apps for my 5s and ran out of room (16 vs 32 gigs). Can't iTunes tell the difference between the two phones?

You may need to configure them manually each time you sync them.
Each device should be identified as a separate one (it helps if they have a different device name). Within iTunes you can configure what you want each device to sync when it's connected to your computer.
