Shuffle randomly selects itself when connecting to bluetooth, help!

Ok, this is my first question. I've owned multiple iPhone models and currently have a 6. I've had the phone since a few days after release. I've owned the same 2012 Toyota Sequoia for three years. About three weeks ago, the phone began randomly choosing shuffle when the bluetooth connects. Happens about 85 percent of the time. This is new, for years the bluetooth audio would just pick up where it left off. I feel like I've tried everything...

Shake to Shuffle is turned off.

I've forgotten the device, I've deleted the phone from my navigation system.

I've re-installed the phone and the vehicle.

I work at a Toyota dealership so I've paired my phone to other vehicles and it's doing the same thing.

I've done a hard reset.

I've deleted all of my music and added different music.

I'm out of ideas. I've heard of other makes (Mazda for example) having issues using shuffle, I'm getting the opposite. I hate shuffle! I like things in order. Driving me nuts.
