It sounds like you and your caller/callee have a low volume when you are on your phone. I would use the troubleshooting in the following article about volume issues on a phone, named:
If you can't hear a person on a call or voicemail or if the sound quality is poor on iPhone

Thanks but I've tried all the possible troubleshooting steps i know and nothing helped. it completely went to even affecting the speakerphone too.

Jun 4, 2015 7:01 AM in response to anabelle20by Lawrence Finch,Then you most likely have a hardware problem. The phone is still in warranty, so take it to an Apple store and they will diagnose it, and most likely replace it.
