When I (or an application) wakes my phone the touch screen is unresponsive, however if I wake, lock, and wake again the screen becomes responsive and I can go about my normal activities, any ideas?

So if I understand you correctly, it sounds like your iPhone's touchscreen is not responsive when awoken, but if woken a second time it works normally, is that correct?
I know you said you have already done a factory restore, I presume you followed the steps in this article?
Use iTunes to restore your iOS device to factory settings
I would suggest then that the next step would be to erase the iPhone and set it up as a new device, without restoring anything from backup. Use the steps listed in this article -
