headphone jack "forgets" my headphones

I frequently have to reinsert my headphones to get my 13" macbook pro running updated Yosemite to recognize them. It will play music out of the built in speakers instead. Sometimes this is after I first plug them in, but sometimes it forgets if I switch from built in display to Thunderbolt, etc. Is this a recognized issue? Is there any fix?

MacBook Pro (Retina, 13-inch, Late 2013), OS X Yosemite (10.10.3)

I am experiencing this problem whenever I'm using a (non-Apple) external display via Thunderbolt, put my display to sleep (when I walk away, for security), and come back. It looks like when the external display wakes up, the Output setting in Sound preferences says, "Oh, there's a Dell monitor that supports audio. Let's send the sound there."

Workaround 1: If your external display has an audio jack, try plugging your headphones in there.

Workaround 2: When you wake up your external display, go to Sound preferences and set Output back to headphones.

Workaround 3: Unplug headphones, reinsert.
