Phone hacked into

How do you detect and remove someone who has hacked into YOUR phone, has sent messages that appear to others came to you but actually were not from your phone. The receiveer of the message said it appeared "layered"on top of another message I had sent. The message did not come from my phone.

In order to have "hacked" your phone, someone would have had to have physical access to your phone. Even then, if you had a passcode on the phone, they would have needed that to get into the phone.

What sounds much more likely is that someone, possibly someone who knows you, has your Apple ID and passcode. Change your Apple ID password immediately.
