My wife and I both have apple watches. We share the same itune account. I get an error message when I try to send her a heart beat or draw a picture. Is the problem that we share the same itune account ?

This is most likely the culprit. Apple don't recommend using the same ID account.
There is an option to trigger family sharing, and with this you share the same bank account CC and also share all your movies, music, audio apps etc.
Downside is if you want to share contacts then one of you will lose that, your wife for example would have to set up your her own contacts details.
