Look at the card. It should have the ICCID printed on it. Does it have 20 digits? If it does, then either the card is defective or the phone is unable to read it, which could be a hardware problem on the phone. But it appears to be reading the card; it is unlikely that it would do a partial read. It should be all or nothing.
Go to Settings/General/Reset - Reset Network Settings. If that doesn't fix it restore iOS on the phone using iTunes.
Use "search" option, here in forum. A lot of threads already open about this problem, today.
Don't panic. It seems it's a Apple issue, related with servers or iOS update.
Try to check again in a few hours or tomorrow. I think they will solve the problem, sooner or later.

The problem might be because of the activation servers being offline or else.
I have the same problem on an iphone 4, unlocked, just restored, and i have tried 3 differents sim cards all give the same problem and if you check forums we arent the only ones reporting this problem.
