How close to the iPad is the subject you're photographing? Which app are you using to take the picture and view it?

In any case, try a simple Reset to see if it helps: hold down the Wake/Sleep and Home buttons until the Apple logo appears.

Get help with the camera on your iPhone, iPad, or iPod touch - Apple Support

Tap anywhere to focus. The camera will not focus if it is too close.
