Connect your old iPhone to computer and launch iTunes on your computer.

Select iPhone icon on the top side.

Press"Summary"tab and tick "This computer" to select "Back up Now".

Then, connect your new iPod to iTunes on PC.

Press"Summary"tab again and choose "Restore backup"

That it is! Your old iPod nano data would be transferred to the new one.
