After reviewing your post, it sounds like you had a ringtone you created, did an update and the ringtone is now missing. I can imagine it must be frustrating. I would recommend that you read this article, it may be able to help the issue.

I made this one from scratch; it is in homage to my great-grandfather, a march music composer in Sweden in the 1800s, from the only recording I have of it.
Luckily I had sent a copy of the entire performance to my daughter, who emailed it back to me. I will edit it down to 40 seconds in Audacity.
What format should I save it in, and how do I get it into my iTunes and thence my iPhone 5?

you will recreate the ringtone but want to know what format to save it in. In the menu you should have the option to add file to library, you should be able to check Tones to sync.

Select MP3, AAC, or AIFF and chose a quality.
