I am assuming that the AirPort Express in question is extending another Apple base station ... correct? If so, what is the exact models numbers of each unit.

3 years is a bit early for issues, but it's not unheard of. Most likely, there are either / or the following that may be the culprit: 1) Wi-Fi interference and/or 2) The current physical placement of the Express is not ideal.

For the former you would want to determine what may be the potential causes of interference and try to eliminate them. For the latter, please take a look at the following AirPort User Tip for additional assistance.
