My wife's phone is missing the Contacts icon, and it's driving her crazy.
We've restored the most recent iCloud backup, and the contacts are there. She can pull up her contacts with the Phone or Email app.
I've also gone to Setting/General/Reset/Reset Home Screen Layout. But that did not restore the Contacts icon.
There are no Folders on the home screen where the icon could be hiding.
What am I missing???

I did as instructed above reset the screen but there is no address app it does show in the configuration with all the other apps.. how do I get it back on the home screen ?
If you reset the home screen layout it is in the Utilities folder, as I posted 2 years ago. If you think you may have deleted it (a feature of iOS that was added after I posted 2 years ago) see: Remove built-in Apple apps from the Home screen on your iOS 10 device or Apple Watch - Apple Support which also has instructions for restoring apps that were removed.
NOTE: There is no "address" app, and never was. It is called "Contacts"
could it be that it was block in restrictions ?
Restrictions are not enabled (OFF)...
If you restored the home screen layout it is in the Utilities folder. It is not possible to delete any of the built in apps, so it is somewhere on the phone. Launch the Search (drag your finger down starting in the middle of the screen) and search for "Contacts". The listing for it will show the folder that it is in.
