My Watch Can't Find my iPhone!

I switched iPhones (from a 5S to a 6 Plus), unpaired my Watch from the 5S and paired it with the 6 Plus (I think), and now my Watch keeps showing me the unconnected icon (iPhone with the red diagonal through it). What can I do to confirm the pairing and get the Watch to talk to the iPhone?
