Forgot Apple ID... etc

Hi

my mother-in-law forgot her Apple ID, Apple Id password, answer to Apple ID security questions and email address and password. Basically everything. We listed and tried all possible usernames and passwords but no luck. What do we do? Her phone is an iPhone 4s and oh her sim card was blocked so she ended up unsubscribing to her carrier. Unfortunately that same mobile number was used as recovery phone number. what do we do?
