iPhone keeps crashing

I have an iPhone 6 16GB and have had it about 4 months. I have had nothing but issues with the phone, it is constantly freezing and restarting. I have had the "Blue Screen of Death" and I just today I had lines where text should be on all screens. Is this a hardware issue and will Apple give me a replacement?
