The disk errors suggest the possibilities that either your disk needs repair via Disk Utility, or maybe the disk is failing. Either of these possibilities could explain the behavior you observe. To check out the former, startup in recovery mode by holding command-r at startup. Then select Disk Utility, then repair your disk.

So if you can recover your disk as CT suggests you should bump your RAM to 8 GB and install an SSD. I've done this on 7 machines so far the earliest, a 2006 iMac and here's my personal early 2011 MBP 13" w/ 8 GB RAM and a 120 GB SSD 6 G all from OWC:
