So I installed iTunes 12.2 on a Windows machine and a Mac and there's a big difference. In the Windows version, I can only "heart" songs. The 5 star rating system is gone. I checked the Mac version and the 5 star version is present and working.
Is it only me or did Apple actually mess things up with the Windows release?
In the Songs view, enable artwork for all albums if needed, then expand the artwork column so that it is wide enough to show the album rating. You can change or set manual ratings from this view.
To eliminate auto-ratings see the scripts mentioned on the previous page.

UPDATE: I've noticed that the ratings are gone in ALBUM view, but present in SONGS view. On The Mac it's available for both.
I normally only use song view...but checked the album view and the "star" ratings are still there for songs and able to be changed.
Same here after upping to 12.2 and OS X 10.10.4 all of the ratings carefully applied over the years are gone!
After a third restart, there they were. Yes!
I am also having this issue. I am also seeing grayed out stars that I have no idea what they mean or how to get rid of them. I used to be able to right click and add/remove a star rating. That is no longer there. The black stars are ones that I have added. The grayed out stars are new and they show up on A LOT of my music and is messing up my smart playlists. . See screenshot.

I just checked on my mac, and the right click rating option is still there. It is missing in Windows. - Also on my Mac those gray stars are not there. Something is definitely wrong with the Windows version.
I'm having the same issue. When I turn on icloud, i see my old ratings along with thousands of extra "ghost" ratings that are sort of grayed out. I've tried everything I can think of to remove them, but haven't had any luck so far. This is screwing with my playlists as well. Apple, please fix this!
Those grey stars are known as auto-ratings. I have some Windows scripts that can hide or restore them:
I see ratings in the songs view if I enable the columns, and the right-click context menu gives me an option to edit the rating.
At this point in time I have a subscription to Apple Music, but I have not enabled iCloud Music Library for my main library, although I have a small test one to see how it behaves.

As you can see from my screenshot above, the right click option to edit the rating is not there in the Windows version.
This problem seems to stem from auto rated album ratings. If you enable the "album ratings" column and then remove all of the album ratings (by manually sliding right to left to get to 0 stars), your playlists should go back to only showing starred songs.
This process could be a ridiculously tedious one if you've got a large library.
But it is there in mine. 😕
It may be related to iCloud Music Library, which I haven't enabled on this library, or it could be a glitch in your install. Note you can also click directly in the rating column to set or clear a rating (but not an auto-rating).

Yes it is the auto ratings that are giving me issues. And they are tracks I've added from Apple Music. I do have the Apple Music enabled. On my Mac it seems to work and the playlists seem to be fine on my phone...
I managed to fix it. Turns out that on Windows, Apple has "Hearts" only as default. Just go to Edit -> Preferences -> General and for Ratings, choose Stars or Stars + Hearts.
EDIT: It's actually called "loves", not "hearts"... lol.
Ahhh that brought back the right click - rating option. But I still have those "auto rating" grayed out stars and this does not let me remove that. I guess I will have to use the scripts for that. Thanks
