iPhone Receives Unexpected Copies of Messages Between Others

iPhone A sends Text to iPhone B, but iPhone C gets a Copy

Dad sends a sensitive text to mom (both have iPhones).

Daughter then sees the message on her iPhone as a message she wrote and sent to her Mom (this should not be possible!).

Mom: iPhone 5s probably running iOS 7

Dad: iPhone 6 plus running iOS 8.4

Daughter iPhone 6 runnning iOS 8.1.3

I looked at: Settings->Messages>Send & Receive. The Apple ID is mine (my daughter is a minor and we haven't set up one.

Under both "YOU CAN BE REACHED BY MESSAGE AT" and "START NEW CONVERSATION FROM" my iPhone number is listed and all my email addresses.

My wife's phone is not listed.

There are no "Text Message Forwarding" items active.

Any help appreciated. Mom and Dad can't text each other until this is resolved..
