For each individual song, you have to choose the 'make available offline' option... I know it's a pain, but I have not been able to figure out any other way of doing it.
This is the same issue I'm having. It's absurd. I paid for 128 GB for a reason.

There is a little icon (picture of phone) that appears either next to each song, or next to the playlist or album title at the top of the display, which shows you that this was made available offline. I think there is confusion here about how this works, but from a bit of testing I think it's probably as follows:
- When you select "Make available offline" on an individual song, you are telling the app that you want to retain this song continually, regardless of anything selected at the playlist, album, or artist level. (until and unless you specifically delete that song).
- When you select "Make available offline" on a playlist, album, artist, or whatever, it does download those songs and you can play them offline, but it doesn't mark the individual song as downloaded. Then, if you select "Remove Downloads", it actually deletes the downloads. If you have specifically marked the individual song as available offline, it won't delete it from your device.
- Any songs that were downloaded before you upgraded your device to IOS8.4 will already be marked individually as available offline - this might create the impression that some playlists have random songs with the downloaded icon in them.
So, if you've made the whole playlist as "available offline", you shouldn't necessarily expect every individual song to have the downloaded icon on it. It has still been downloaded.
I thought this was an issue at first but after thinking about it more, it seems to make sense to me. If I delete the downloads on a playlist, I still might want to control that certain songs are not deleted.
Now, if you're saying that you've done all this, and you can then prove that your device is still streaming the songs even though they've been downloaded, that would definitely be an issue. I have tested this in airplane mode, but I don't know how to check whether it's wrongly streaming downloaded files.
One last warning - if you have selected a large amount of music to "Available offline", make sure it's all downloaded before assuming you have a problem - this was also confusing me when I was getting a lot of pauses and freezing, but I suspect it was because the downloads weren't complete yet.
I've noticed this issue too. I've been downloading whole albums from Apple Music for offline play to listen to in the car. When I've done so, the small device icon shows on the album art, but not the individual tracks as Patthesplat describes. I have turned off mobile data usage under Settings/iTunes, and as expected once I'm off wi-fi songs are shaded out apart from the ones I've downloaded. Any everything plays fine offline. However, Music still seems to eat through data whilst I'm listening to these downloaded albums!
The strange thing is, if I go to Settings/General/Usage I can see the albums taking up local storage space, and if I turn on Airplane mode these albums do still play, so they are definitely downloaded. It's almost as if they're streaming anyway when connected to mobile data (even with mobile data turned off for Music).
