There are tons of apps in the App Store which can be used for that purpose. Whether or not it's legal is another matter. For the U.S., some states don't allow it by non law enforcement persons at all. Others require you to inform the person you're speaking with that you would like to record the conversation. If they say no, you can't.

Insurance co's should be asked to provide it in writing. Chat scripts should be emailed to you. Recording such calls is futile as a human will misinterpret documents.

Depends on which ones you click on. I found plenty that will record both incoming and outgoing calls.
