Cannot access phone from slide screen

Just got my phone back from Apple Care on Friday. Restore with update to 8.4 done on Saturday and today it will not allow my to access the phone from slide screen. I've done a hard restart 3 times with no avail. The phone rings, I see messages from apps but no access. Also something interesting is when my husband test called me, it only displayed his number, not hid name. I've tried restoring from last back up, but iTunes will not allow the restore since Find My iPhone is enabled.
