Only the carrier it is locked to can unlock it.
You must contact the carrier, and see if you fulfill their requirements for unlocking.
Actually, it was a Verizon phone originally, and I just learned that Verizon doesn't lock its phones.
Then if the original seller won't take it back for a refund, maybe you can sell it to someone who is amenable to being a Verizon customer.

That is correct. Any of the Verizon phones that were LTE capable, like the iPhone 5, the GSM side of the phone is unlocked. You will not need to unlock it to use it with another carrier. The requirement to unlock the GSM side of their LTE phones was a settlement for obtaining additional LTE bands. You should be able to just insert the SIM for the new carrier you wish to use and the device should work.
Correct. you should be able to use the GSM portion of the iPhone on any network. you will not, however, be able to use the CDMA portion on anything but Verizon.
