accidental Find My iPhone removal
Find my iPhone on the iPad. All devices appear except add iPhone. I had it added originally but hit "remove" by accident. Any way to restore that?

If I'm reading your question correctly, you were signed into icloud.com and under Find My iPhone you removed your iPhone 5. From your iPhone go to Settings | iCloud and turn Find My iPhone off and then back on. See if that will allow you to see it on icloud.com again.
