EFI security update 2015-001 keeps appearing in updater

I am running 10.9.5 on a early 2011 MacBook Pro (8,2). I have tried to apply EFI security update 2015-001 from the app store updates. The update downloads and appears to execute correctly (and without notice). After I get the long tone, the machine reboots but neither my Boot rom nor my SMC versions are different. Moreover, the App Store checks for updates and the same EFI security update 2015-001 appears as needing to be installed. I have tried this with the machine disconnected from all but power and internet cable. This history indicates that this EFI security update has been installed. The hardware details are below. What does this behavior mean? Was it already installed ? is the machine still vulnerable?
