I've just started using the app Passbook. I am trying to scan several grocery rewards cards (like A&P, CVS, etc) into the app using my phone. But I keep getting the message, "No Pass Available for this code." Do I need to put them in an app like KeyRing first?

Passbook only has the option for certain cards and currently there is no way to manually add them. With iOS 9 Passbook will be renamed to Wallet and there will be more options to add store rewards cards and such but my thought is that the retailer will still need to work with Apple to enable it.
