I cannot get my iMac to start up properly. Can someone please help?

I've tried the following:

Regular start: the progress bar stalls

CMD+R start : progress bar stalls

CMD+Opt+R start: after recovery download the apple logo appears

Option start: I select the HD disk and progress bar stalls

Shift start: progress bar stalls

All I want to do is to a clean reinstall of my factory settings and upgrade until I am back to the current Yosemite version.

Apple Hardware extended test reported No hardware problems.

What OS X came on it and what OS X is it running...?

Sounds like it is either not starting up into OS X Recovery or you are missing some step(s) in OS X Recovery.

Carefully read > OS X: About OS X Recovery - Apple Support and then use Recovery to either Repair your Startup Disk or erase and Reinstall the OS X.

Also note: that you will need a good backup if you plan on erasing the drive and starting over.
