Set up a hotspot by:

Going to Settings > Wi-Fi on your other iOS device and select your iPhone or iPad that has a cellular connection.

Have you clicked on the link Carolyn provided? Have you contacted your carrier to ensure that the personal hotspot is a feature they offer and that it's enabled on your account?
