Mid 2012 Mac Pro 5,1 frequent kernel panics

Starting from March, I've been experiencing more and more kernel panics. To the point that it would occur more than 10 times a day. I decided to reduce peripherals to a bare minimum and do a clean install of the OS (Yosemite). It was good at first. After a couple days it happened again. I took it to the Genius Bar later. The first time they replaced my graphics card. I took it back home and did another clean install of Yosemite. It still happened. I took it back to the Genius Bar again. This time they replaced the logic board. Took it back home. Did a clean install of Yosemite 10.10.4. Did not install anything else. Let the machine sit there powered on. I only browsed the web lightly. After 2.8 days a kernel panic happened.

I'm puzzled. I've heard stories such as someone had his MBP logic board replaced twice before his kernel panics finally disappeared. I'm tired of making these trips to the Genius Bar. Can someone help me figure out what's going on? I did have a USB TV tuner (EyeTV One) plugged in but I didn't install the associated software. Also previously I had had kernel panics without the tuner plugged in. My mouse is a bluetooth one but the receiver of the mouse signal is a usb device that's plugged in the usb port under my Apple keyboard.

Here's my latest panic report (new graphics card and new logic board):

One thing I forgot to mention. The first time I took the machine back home from Apple Store (after a few days' wait), it was an evening, I powered it up and found there was a kernel panic report generated at 7am that morning. But I remembered clearly they had called me around 10am telling me the machine was ready to pick up, that is, only three hours after the kernel panic. Odd. Maybe they decided to replace the graphic card after the kernel panic. But then at least they didn't test the machine after the change, at least not long enough.

Thanks Linc for the advice. I have just replaced my Logitech bluetooth mouse with an ordinary wired mouse. The TV tuner is unplugged from the usb port as well. What's interesting to report - last night after I disconnected the TV tuner from the USB port, I decided to experiment with something I had a suspicion on due to prior experience. That is, playing a Youtube video and a Comcast movie simultaneously. The Comcast movie player needed the Adobe Flash player. Installed that. This was the first thing I had installed on top of Yosemite. I randomly picked a movie and a Youtube video and played the two simultaneously. Kernel panic happened 5 hours later. I previously had kernel panics much sooner than 5 hours, more like within 15 minutes. So when this latest kernel panic happened, I had a bluetooth mouse plugged into a usb port, and flash player installed. Here's the panic report:

Thanks Linc. Another kernel panic last night. This time around there was no TV tuner or bluetooth mouse. I was playing one Comcast movie (on Safari) and two Youtube videos. Here's the panic report:

This panic is "Spinlock acquisition timed out" on its face is a multiple-processor problem, including Graphics processors. But the synchronizing element is word In RAM Memory.

If your Mac Pro with Xeon Processor has had its Hard Drive erased and Mac OS X re-installed from scratch, it may have a Hardware problem.

If your Mac did NOT have Error Correcting Code memory, this set of panic logs indicates a Non-Trend. Either memory problems, or subtle processor problems that are not being caught by diagnostics.

Did you try the original RAM, as Linc Davis suggested earlier?

Because of the way the third and fourth slots on each side are "shared" in these Macs, having the fourth DIMM present on each side becomes an area of interest. Adding the fourth DIMM changes Bus loading and memory timing.

Have you considered getting some at least one UnRegistered 8GB DIMMs for each side and using fewer of them (especially leaving the fourth slots on each side empty) ?? (This changes memory bus loading and memory timing.)

a "cheap" test would be to leave the fourth slot on each side empty, and see if the panics subside.

Grant, I appreciate your help. Yes, I do have ECC memory. I bought them from OWC. My System Report also says that I have 8 DDR3 ECC DIMMs running OK. I have erased the hard drive and re-installed Yosemite a few times now. The panic I reported in my first post (invalid opcode) happened in a freshly installed Yosemite, with no other software/driver installed, including Adobe flash player. System was left idle, with Safari sitting at home page of apple.com. That panic happened 2.8 days after boot, during which time I just lightly browsed the web once in a while. I can probably live with 2.8 day uptimes, but I'm sure if I don't fix the panic issue, it'll just progressively get worse.

I'm not sure if the RAM modules I replaced with the current 4GB modules are Apple memory. The labels on the modules say "hynix/KOREA 08" etc. They're 2GB modules. Six of them altogether. Actually the reason I replaced them with 8 x 4GB modules was that I was experiencing mysterious kernel panics. So back then the fourth DIMM positions were not occupied.

Since I have scheduled to take it to the Genius Bar again tomorrow, I'll temporarily put further tests on hold. I will report back after I get it back from Apple Store. Thank you so much Grant and Linc. I love the community.

Got my machine back from Apple Store two days ago. They told me they'd run some thorough tests and found nothing. They reinstalled my OS ("Level 1" software fix). I needed to use the machine so I installed Adobe CC and Blackmagic Desktop Video (also a Decklink video capture card). The machine had run continuously for 32 hours (mostly idle) until a kernel panic occurred this morning. I know third party software can always be an issue but I have this strong feeling that the machine isn't fixed. And I have this strong feeling that kernel panic will happen more and more often as I further burden the machine with more software and/or continue using the machine. Frustrating. Here's the latest panic report.

Just in case someone is following this thread. I think I finally had my machine fixed. It's been running continuously for 17 days, with lots of third-party software loaded, and not a single kernel panic has occurred. What happened is that Apple support team finally decided to escalate the issue and sent my kp reports to their engineering team. Word came back a few days later that it was the CPU, because the kp reports said "CPU(s) failed to respond to interrupts." So they replaced the board/module that the CPUs plug into, as well as one of the CPUs. So far I've been testing with the 12GB memory. I'll put back the 32GB memory shortly. I have little worry that that might cause any trouble.

So in my case, they replaced the graphics card, the logic board, the power supply, and finally the CPU board and one of the CPUs. It was the CPU and CPU board replacement that did the trick. In retrospect, I think it might be the CPU board that was the culprit because my other CPU (the one they didn't replace) also had "failed to respond to interrupts", just that the engineering team wasn't sent that kp report.

It was a long process of lots of trips, phone calls and online chat sessions. I had Apple Care so it cost me nothing other than my time and frustration. I'm glad it has finally been fixed.

Thank you for posting back with your results -- that you persisted in pressing your case with Apple and they eventually escalated and solved your problems under Applecare at no repair cost to you.

The forums are a lightning rod for problems, and many of those go cold without final closure here. We often don't know for sure whether they got resolved or the User gave up.

Thank you Grant for trying to help out. Another good thing about Applecare I found out is that for big machines like the Mac Pro they actually can come to your home to do the repair, if you insist. I found out a bit too late, right before the last repair effort. Saved me two trips, though.
