Mac Mini(2012) Duel Monitors (4K & 1080p)

Hi.

I have a Mac Mini Server (late 2012), 2,6 GHz Intel Core i7, 16GB DDR3 Ram and Intel HD Graphics 4000 1024MB.

I also have a Samsung 34" Wide UHD Monitor (3440X1440) (Primary Monitor) and a Samsung 27" FHD 1080p Monitor (Secondary Monitor)(1920X1080)

My Goal is to get both monitors running on my mac mini.

I connected the Samsung 34" in to the Thunderbolt Port via a Display Port Cable with a thunderbolt Connector that came with the screen. And I connected the Samsung 27" with the HDMI port with a HDMI cable. If a set the resolution on the Samsung 34" to 3440X1440 and the Samsung 27" to 1920X1080. Then the bottom of the Samsung 34" Screen is blurred and also where ever I move the mouse it has a blurred square about the size of 2 match boxes. So the mouse and everything under it is not visible.

Does anyone know what the problem could be?

Okay so after struggling a lot with this I think I am about 50% there. I searched other forums and came across this thread. Lost ability to choose 3440x1440 resolution. I followed Richard Delph instructions and used his download link. For some reason the other one would not work. It worked perfectly I got my 3440X1440 on my Samsung 34" monitor back. On any port HDMI or Thunderbolt.

Now when I plug in my Samsung 27" Monitor I get this distorted image as attached. The image shows the screen of the 3440X1440 (Samsung 34") and at the bottom you can see the second monitors menu is starting to show. There is also a distorted section like this on the top of the second monitor (Samsung 27").

Does anyone know why this is? I saw on a post that they change the vram form 512mb to 1024mb. My mac mini came with 1024mb. So any suggestions I can try?
