MacBook Pro WiFi Goes Off & Won't Come Back On

I have a MacBook Pro, 15", model A1286. I am running OS X Yosemite version 10.10.3.
Whenever I turn it on, the WiFi works fine for 2-5 minutes and then goes off.
It shows that it is still "on" from the menu bar icon (i.e. all bars full), but when you click on it it appears that it is off because it has the menu option to "Turn WiFi On."
So then if you go into System Preferences > Network, it also shows that WiFi is off, and if you click the button nothing happens.
I have tried to "Make Service Inactive" on WiFi, then remove it, and then try to add it again, however, my computer freezes and eventually crashes after trying to do so.
Interestingly, if I close the lid, leave it, and come back it shows now that WiFi is on (blank bars like unconnected in the menu bar), however it shows that there aren't any networks to connect to. I can then turn WiFi off, then turn it back on, and it will reconnect to my network, but then again will stop working after 2-5 minutes, but it will still say it is on. I can turn it off, and then can't turn it back on.
And just for everyone's information, I have plugged in my laptop directly with an ethernet cable, and it works just fine, so it definitely is something with the WiFi.
Is my WiFi card bad? Or is it something I can easily fix?
