There's no away to do this. You can add musics on iTunes and sync to your iPod and iPhone or sync by wifi. If you don't want to sync, you can simply subscribe to iTunes Match and you can listen yours musics from all devices (iPod 5th). The musics will be saved by icloud.
Syncing is a one way affair except for purchased items from the iTunes store.
What happened to computer the iPod touch was syncing to, and got the music from?
There is no direct way to get music out of an iPod. To get the music out of the iPod you will need a 3rd party tool. You can then import it into the iTunes library on the Mac, and then sync it to the iPhone.
iTunes Store purchases can be transferred to a computer.
iTunes Store: Transferring purchases from your iOS device or iPod to a computer - http://support.apple.com/kb/HT1848 - only purchases from iTunes Store
For transferring other items from an i-device to a computer you will have to use third party commercial software. See this document by turingtest2: Recovering your iTunes library from your iPod or iOS device - https://discussions.apple.com/docs/DOC-3991
