How do I get this correctly formatted for Pages?

Hey everyone,

Ive been trying to get this to show up on a single Page in Pages but it always splits the table. I would greatly appreciate any help on this!

Im trying to use this as pages since I can then edit this on my Phone and also have a copy on iCloud.

I do not want to use word for my iPhone.
