safari still autofilling for all sites even after empty cache, clear all history, and restarting

I have emptied the cache, cleared all history, closed/reopened Safari, shutdown/restarted the computer and every website is still opening with autofill information. Can you please help me to really empty the cache and clear the history? I am running OS X Yosemite, version 10.10.4. Safari version 8.0.7 (10600.7.12). Thank you.

MacBook Pro, OS X Yosemite (10.10.4)

From your Safari menu bar click Safari > Preferences then select the Autofill tab.

Deselect the box or boxes then quit and relaunch Safari.

If you don't want passwords to autofill, select the Passwords tab then deselect: AutoFill usre namesand passwords
