Playlists disappeared from phone; no iCloud option under music settings

iTunes version: 12.2.1.16

iOS version: 8.2


Hello, I recently wiped my computer clean and reinstalled iTunes. I have been able to preserve all my playlists and other metadata; unfortunately, when I synced my phone to the computer, all of my playlists disappeared from my device. The folders containing playlists have been 'flattened' into one single playlist.-- all my organization is gone. Some of these folders contained over 100 playlists.

The playlists are in tact on my computer, however.


The solution to the problem is to go into Settings > Music > Turn off iCloud music-- but that option does not show up for me. How do I get this option to appear, so I can get the playlists to appear on my phone?
