What is current thinking on app title lengths & violation notice with no reason given

Apple gave us 255 characters for app titles originally then it seemed to be reduced to 100 about a year or so ago and now in Appendix B of the iTunes Connect Developer Guide, Appendix B there is mention of no more than 75 characters. Does anyone here know with 100% certainty that 75 characters is the new app title length limit? And has anyone had any older apps pulled by Apple from the app store (with no notification on their part) because of somewhat lengthy titles that Apple had approved as late as last year? And when I say lengthy - not talking about 255 characters but just a bit over 100 characters...

And has anyone gotten a notice of violation from Apple with no reason given? And if so - who did you contact to get more information and to get it resolved?
