HELP asking again: I have ? in my chapter titles, but only some of them show up in the ToC, why?

I have question marks in some of my chapter titles, but not all of them show up in the table of contents.

I've retyped them, deleted and retyped them, extended the text box, checked image placement. Nothing works. All the same font and placment.

Using 2.3 and MakBook Air. See below. 1.2 and 1.3 should have ? This image is from the ToC.

Any ideas?
