Only you can answer the question about Apple Care. It extends the warranty and provides additional coverage. Review the additional coverage provide and then decide if you think it is worth it or not.

This is incorrect. You may only purchase AppleCare+ at the time of purchase or within 60 days of your Apple Watch purchase.

If you cancel AppleCare+ within 30 days, you will receive a full refund (assuming that you have not made any claims or received any services under AC+).
Even after that, you would be entitled to a pro-rata refund* (again assuming no claims / service received).
