EEvery time I want to connect my iPad mini to my iPhone 5 via personal hotspot it won't recognize my phone. If I change the phones name is allows me to establish a new connection and all is good. When I disconnect and try again I have to change the phones name again.

It seems like you have to repeatedly change the name of your iPhone in order to connect to the Personal Hotspot with your iPad mini.
