Go to Settings > General > Accessibility and check settings there. Perhaps the Voice Over will help.... or the Speech selections ?

Many other options are available. I would call AppleCare or drop by an Apple Store if you have one close by as they can surely help...

There are lots of options. The iPhone supports switches of various types. Review the section of the manual on "accessibility". Or, better yet, have your friend's occupational therapist review it.

You could try talking with their physical or occupational therapist if they have one and see if they can try out something called switch controls. These can be mounted to a headrest of a wheelchair and activated with the side of the head or even with buttons and used with little hand control. Voiceover will not work with paralysis. It requires double tapping the screen and multiple other gestures including three finger double taps etc. VoiceOver is screen reader meant for people who are blind or have low vision, not paralysis .
