Neither of these options have helped. When I start up the apple sign is on the screen then a upload bar starts and only gets to approx 40% then ***** down immediately.

If you completed all of those steps, try to boot to your recovery by holding CMD+R on boot up, then reinstall OS X. OS X Yosemite: Reinstall OS X
